Compounded tirzepatide vs. compounded semaglutide: how clinicians choose
Both molecules act on the GLP-1 axis. They are not interchangeable, and the choice between compounded tirzepatide and compounded semaglutide is not a coin flip — it is a clinical decision shaped by mechanism, tolerability, comorbidities, and the patient’s history. Here is the framework Form’s clinicians actually use.
Before anything else, the regulatory line: compounded GLP-1 is compounded under section 503A of the FD&C Act; compounded GLP-1 is NOT FDA-approved as a finished product. Neither compounded semaglutide nor compounded tirzepatide is the same product as, or interchangeable with, any branded finished GLP-1 medication. Anyone telling you the molecules are “the same thing in a different vial†is either being careless or selling you something. With that out of the way, the differences worth understanding are mechanistic, pharmacokinetic, and clinical.
Mechanism: one receptor versus two
Semaglutide is a GLP-1 receptor agonist. It mimics endogenous glucagon-like peptide-1, binding the GLP-1 receptor in the pancreas, gut, and central nervous system. The downstream effects clinicians care about for weight management are slowed gastric emptying, increased post-meal satiety, reduced ad libitum food intake, and modest improvements in glycemic control through glucose-dependent insulin secretion.
Tirzepatide is a dual agonist. It binds both the GLP-1 receptor and the glucose-dependent insulinotropic polypeptide (GIP) receptor. GIP is the other major incretin hormone, and the addition of GIP agonism appears to amplify several of the metabolic effects of GLP-1 alone — particularly satiety and post-meal glucose handling. In the published phase 3 trials of finished branded products, the magnitude of total body weight reduction observed with the dual mechanism has been larger on average than with GLP-1 monotherapy. We mention that as published clinical pharmacology context; it is not a claim about compounded preparations.
Titration: the schedules are deliberately slow
Both molecules require dose titration because the gastrointestinal side effects of GLP-1 receptor activation are dose-dependent and adaptive. Starting at a low dose and stepping up over weeks lets the gut habituate. For semaglutide, the conventional clinical titration starts at 0.25 mg weekly and steps through 0.5, 1.0, 1.7, and 2.4 mg over roughly four to five months. For tirzepatide, the corresponding ladder starts at 2.5 mg weekly and steps through 5, 7.5, 10, 12.5, and 15 mg over a similar window.
Two practical implications:
- The first few weeks are not “the treatment.†They are the on-ramp. Appetite changes are usually mild at starting doses and become more noticeable at the second or third step.
- Patients who escalate too fast usually pay for it. Skipping steps or doubling early to chase a result tends to produce the nausea and constipation that lead to discontinuation. A slower, well-tolerated titration outperforms a fast, intolerable one.
Side-effect profile: where they overlap and where they diverge
The side-effect profile of the two molecules overlaps substantially because the shared mechanism is GLP-1. Nausea, decreased appetite, constipation, diarrhea, eructation (the famous “sulfur burpsâ€), fatigue, and transient injection-site reactions appear with both. Most are mild to moderate, dose-related, and improve with time on dose.
Where clinical experience and trial data suggest divergence:
- GI tolerability at matched intensity: some patients tolerate the dual agonist better than expected at higher metabolic intensity, possibly because GIP partially offsets the gut symptoms of GLP-1 monotherapy. Others do not. There is meaningful individual variation.
- Glycemic effects: tirzepatide tends to produce stronger A1c reduction in patients with type 2 diabetes, reflected in its diabetes-indication trials. For non-diabetic adults pursuing weight management, the practical implication is smaller — but the metabolic effect is real.
- Gallbladder-related events: rapid weight loss of any cause raises the risk of cholelithiasis. Both molecules can be associated with gallbladder events in patients losing weight quickly. Clinicians watch for right-upper-quadrant pain, particularly with fatty meals.
How clinicians actually choose
In practice the choice is rarely about which molecule is “stronger.†It is about fit. The questions a Form clinician is weighing during a first visit include:
Metabolic context
For a patient with prediabetes, insulin resistance, or type 2 diabetes, the dual agonist mechanism of tirzepatide is often the more interesting starting point, because the glycemic benefit is more pronounced. For a patient whose primary concern is appetite regulation and whose metabolic labs are otherwise unremarkable, semaglutide is a perfectly reasonable first choice.
GI sensitivity and prior experience
A patient with a long history of severe constipation, slow motility, or a documented gastroparesis diagnosis is generally not a good candidate for either molecule. For a patient with mild reflux or a sensitive stomach, the conversation centers on starting low, titrating slowly, and front-loading hydration and fiber strategy. Prior experience matters too — if a patient previously tolerated semaglutide well but plateaued, switching to tirzepatide is a different decision than starting fresh.
Patient preference and access
Some patients have a strong preference for one molecule based on what a family member or friend used. Clinically these preferences are not decisive, but they affect adherence, which is decisive. The clinician’s job is to make sure the preference is based on accurate information rather than marketing or a friend-of-a-friend story.
The safety floor that does not change
Whichever molecule a clinician and patient land on, the contraindications and precautions are the same. Personal or family history of medullary thyroid carcinoma or MEN-2 is an absolute contraindication. A history of pancreatitis warrants serious caution and an honest conversation. Current or planned pregnancy, breastfeeding, severe gastroparesis, and active eating disorder pathology are also stop signs.
Boxed warning: Risk of thyroid C-cell tumors. Do not use if you or a family member have a history of medullary thyroid carcinoma (MTC) or Multiple Endocrine Neoplasia syndrome type 2 (MEN-2).
